How to Get Started
Here is a basic list of most of the documents and information that are required for the admission process:• History and physical
• Face sheet/patient demographic sheet
• Primary care provider chart notes (last 3 months)
• 1 month of nursing notes (if coming from an outside facility)
• Any current labs/X-rays
• POA/Conservator/Advance Directive/POLST (Not Required, but recommended)
• Copy of all insurance cards
• Copy of Social Security card
• Current medication list
• Immunization record
• Code Status (DNR/DNI/Full Code)
