How to Get Started

Here is a basic list of most of the documents and information that are required for the admission process:

• History and Physical
• Primary Care Provider chart notes (last 6 months)
• Any current labs/ X-rays
• POA/Conservator/Advance Directive/POLST
• Copy of all insurance cards
• Social Security card (copy)
• Current Medication List
• Immunizations
• Code Status (DNR/DNI/Full Code)
• Vaccination Status (Covid-19) and copy of Covid vaccine card
